Overview
Enhabit is hiring an RN or LPN / LVN to join our team as a Hospice Admission Coordinator- Weekend The Admission Coordinator will be responsible for regional coordination of hospice patient referrals. This position will work closely with branch staff to ensure that adequate coordination and transition occurs to hospice services.
This a fully remote position. This position is for Friday at 5 pm - Monday at 8 am.
Responsibilities
- Receive, document, and appropriately communicate referrals from physicians, facilities, and staff
- Gather and review required documentation for the hospice admission process
- Obtain and enter verbal orders from physicians and midlevel practitioners into HCHB; ensure quality care coordination
- Evaluate patients for hospice appropriateness and clinical eligibility
- Initiate contact with patient to verify awareness of referral, to introduce company, and to verify information
- Obtain agreement for admission from referred patients and their care givers
- Follow-up with patients who remain in the hospital or rehabilitation facility until current orders are received for admission
- Verify Medicare eligibility and ensure accuracy of Medicare number in patient record; notify the Admission Director if Medicare is not the primary payor source
Qualifications
- Must have a high school diploma or equivalent.
- Must be a graduate of an approved school of nursing - RN or LPN / LVN
- Must have one year of demonstrated experience in hospice. Three years of experience in hospice is preferred.
- Demonstrated knowledge of Medicare guidelines governing hospice is required.
