Responsibilities

Enhabit Home Health & Hospice is looking for a Licensed Master Social Worker (LMSW) to provide professional, comprehensive, family-oriented services to individuals in the home setting.

Enhabit Home Health is hiring an LCSW to join our team in Pueblo. At Enhabit, licensed social workers provide personalized support to patients and their families in the comfort of their own homes or place of residence. Enhabit offers a non-traditional medical setting where social workers can see the direct impact they have on their patients.

• Coordinate with interdisciplinary healthcare teams to enhance continuity of care
• Provide rehabilitative and supportive casework geared toward restoring patients to their optimum level of social and health adjustment
• Help patients and their families understand, accept, and follow medical recommendations
• Assist patients and their families with personal and environmental difficulties that may predispose them toward illness or interfere with their ability to obtain the maximum benefits from medical care

Qualifications:

• Must have a master’s degree in social work from a CSWE accredited program
• Must have a current Colorado LCSW licensure and be in good standing
• Must have at least one year of social work experience in a healthcare setting
• Medicare home health or hospice experience is preferred
• Must possess a valid state driver license, maintain automobile liability insurance as required by law, maintain dependable transportation, and must be able to safely drive in all types of weather
